About Us

Northeast Wisconsin Family Care is engaged in a two year planning process to develop a new entity, called a managed care organization, that would provide Family Care in Brown, Door, Kewaunee, Marinette, Menominee, Oconto and Shawano Counties. The planning process is conducted in accordance with a planning grant from the State of Wisconsin.

Northeast Wisconsin Family Care planning will build toward the ability to begin enrolling consumers in late 2011. As county-by-county enrollment into Family Care begins, individuals currently in county provided Home and Community Based Waiver services will be enrolled to be followed by individuals on wait lists for these services. Family Care will be an entitlement to service within three years of a county’s implementation of the service.

Northeast Wisconsin Family Care has agreed to also work cooperatively with Community Care, Inc. a private, non-profit managed care organization toward their provision of a long-term care service that will provide an option for some consumers. This service, called Partnership, combines Family Care services with management of a consumers’ health care services including physical and hospital services. The Partnership program is an option that consumers with complex or chronic medical conditions may choose in lieu of Family Care.

People eligible for Family Care and Partnership also have an option under Wisconsin’s IRIS program of choosing to self manage their goods and services and may use IRIS to remain in their community and avoid moving into a nursing home or institution. Individuals who choose IRIS have more choices, control and freedom to design their own support and service plans to meet their functional, vocational, medical and social needs.
